Worst game I've ever played?
Pretty much anything new and "critically acclaimed" such as the Halo series, Call of Duty MW series (that includes black ops).
From way back when? I'd have to go with the ENTIRE Deception series by Tecmo. Good concept (kinda) but I found the execution of it absolutely horrible.
Going back further I'd have to say Sweet home. I found how the game works hard to understand. Certain weapons only hit certain enemies, it's not always clear which weapon is better than what and the perminant death, whilst a good indicator of "realism" in a game about a ghost that killed babies because her baby fell in a furnace, just annoyed me more than anything else because each character carries something that makes the game easier.
Fighting through Aliens, Robots, Insane sick people, demons, plants, dragons, fish, squids, jellyfish and more for the good of the forum!
When I play a game, I usually want to see lethal amounts of death and destructive amounts of ruination.

Yeah you can't just play FFVII and be a fan of the Final Fantasy series, you gotta go out there and play the other ones. VII is my favorite but with that said, it is not the only one I find amazing.
Now to games that are down right horrible. Prince of Persia, the one that came out for the Xbox 360 and PS3, not the Sands of Time series. Also, Final Fantasy XIII; I F***ing hated it!

I thought you meant the original Prince of Persia for a moment. That's good fun, if a tad frustrating. As for the new version, I only played about an hour of it but thought it was okay. If I remember correctly, the whole "reversing time" thing was really over-the-top, but overall the game was playable and surely not the worst game this generation. Not by a long shot, unless the gameplay quality dips dramatically soon into the game.
FFXIII, I've already expressed how the fact it shat all over the franchise angered me, although I guess that's Squeenix's fault. Not the worst game I've ever played, but I didn't get to the ten-hour mark. I simply just didn't see it as a Final Fantasy game and it was extremely lacklustre in comparison to most. I sold it to help pay for New Vegas. One of the best decisions I made last year.
